Accreditation for Medical Practitioners

Clinicians who wish to admit patients to Linley Clinic will need to be accredited by the Linley Clinic Board and Medical Advisory Committee.

The following documents will be required:

  • Completed application for Accreditation – Medical Practitioners
  • Sighted and Signed Linley Clinic Medical By Laws
  • A current Curriculum Vitae
  • Copy of Current Registration
  • Copy of Current Indemnity Insurance
  • Working with Children National Criminal Record Check from the GPO
  • National Police Clearance
  • Certified copies of credentials as stated in application
