Preadmission

To book your procedure a Linley Clinic, you will need to phone your Specialist’s Consulting Rooms.  Go to surgeon information

There are two forms that are required to be completed and returned to Linley Clinic a week prior to surgery where possible.  This assists us in preparation for your visit.  These are given to you by your surgeons.  These are;

• Preadmission Form
• Consent Form

Nursing staff will contact you and give you details of Admission and Fasting times as well as confirm a number of Medical details with you before the Business Day prior to Surgery.
